ABSTRACT
Background: The cost of femoral fracture
management, with its increase in frequency constitutes a public health
problem.
Objective: To contribute to the better
management of femoral fracture.
Patients and methods: A retrospective and
descriptive study has been carried out in 204 patients hospitalised and treated
with IMN, in surgical department of CHUK, between January 1st 2004
and December 31st 2006 because of diaphyseal femoral fractures.
Results: During the period of our study, the
number of cases per year has increased to 3.7 times. The ages of our patients
are ranged between 11 and 82 years with a mean of 31.86 years. The big number
was males (80.9%) with a sex ratio of 4.1. RTA accounted for 66.7% of cases as
cause of injury. The site of fracture was middle third of the shaft of femur
(47.1%), and 69.6% of fracture pattern was transverse. 80.4% of our patients
had consulted during the 1st 24 hours after injury. The average time
from arrival to surgery was 20.88 days, and the average LOS was 27.95 days. The
Bohler-Braun splint was the most used as initial treatment (88.2%). Open IMN
was used in 71.1%. The Küntscher nail was used in 97.1% of patients. 7
patients (3.5%) developed intrahospital complications and the outcome was good
in 96.5% of patients.
Conclusion: This study shows the value of
improvement of the management of femoral fractures, especially by reducing the
LOS which results in increase of costs and complications. Intramedullary
nailing is an excellent method of shaft femoral fracture fixation in a
developing country set up.
